V400 volume

My earpiece volume keeps going extremely low at times. I have the gain table
turned up to 4 and volume all the way up. When it does this I can shut the
phone off and remove the battery for a minute and when I power it back up,
the volume is fine.

Any ideas as to what is wrong and how to fix it?
