GSM MAP Application-Context to Operation-Package to Operations

We have added to our Technical Briefcase again.
We created two sets of tables that describe the relationships between
Application-Contexts, Operation-Packages, and Operations within the GSM
MAP protocol. One set of tables presents this information in
Application-Context to Operation-Package to Operation (ACOPO) order, and
the other in Operation to Operation-Package to Application-Context
(OOPAC) order. GSM 09.02 SDVs 7.10.0, 6.8.0, 5.16.0, and 4.19.1 are
covered by separate tables.
