Although there are a lot of GAIT phones coming out covering all the bases for
GMS, TDMA, and AMPS, is there a phone that is going to be a GSM
800/900/1800/1900MHz phone?

Actually there are now five (5) GSM bands! You missed GSM 400. See

The up-coming Motorola V90(i?) supports all these bands.

Is anyone actually using GSM-400? The last I heard, it was in trials to
replace NMT-450 but no one adopted it.

well, someone else mentioned the GSM 400 which will replace NMT..., but
there are a couple of GSM 850/900/1800/1900 phones on the horizon, but
none are available yet.
Motorola V600
NEC 515

Moscow Cellular Communications (MCC) will use 400 and 1800 MHz bands
for building a dual-band GSM network in Moscow city and its
environments. Corresponding license has been issued recently by
Russian State Committee of Radio Frequencies. The company already
began its efforts for the deployment of trial dual-band GSM network
and will proceed to hardware installation in the nearest time. MCC
firmly believes that it will actually prove the subscribers,
investors and participants of the market that GSM 400/1800 is the
optimal platform for migration to third generation (3G) cellular
communication services in Russian conditions.

Another question would be:
Are there any phones supporting GSM400?
